Pricing

Initial evaluations:

$395/90 minutes*

Follow up appointments:

$265/60 minutes*

Telehealth:

Visits range from $135/30 to $265/60 minutes* and pricing is prorated based on time scheduled.

*There will be a 3.5% processing fee added to all credit card transactions.

Travel visits:

A $175 travel fee added to the above pricing for all in home visits

Insurance:

• Reclaim Pelvic Therapy is an out of network provider for all insurance companies. Please contact your insurance company if you have any questions about your out of network benefits. Reclaim Pelvic Therapy will also be happy to provide you with any documents/codes which will assist you in receiving reimbursement.

• Reclaim Pelvic Therapy can provide superbills (in most cases) which patients/clients may submit to their insurance company and/or flexible spending accounts or health savings accounts for out of network reimbursement. Patients/clients are responsible for knowing their benefits if choosing this option.

• Patients are responsible for payments at the time of service via credit card, Venmo, cash, or check.
